Introduction: The aim of this study was to evaluate the Pre-Orthodontic trainer T4K appliance utility in the treatment of mandibular primary crowding in the early, mixed dentition stage therapy. Material and methods: The study material comprised 62 subjects, 42 girs and 20 boys from 7 to 10 years of age. The myofunctional trainer was applied to 21 children with lower incisor primary crowding and mentalis muscle hyperactivity or/and lower lip biting habit. The results of the treatment were analysed by means of clinical evaluation and the dental pre- and posttreatment cast measurements of the Irregularity Index of Little (II). The evaluation of the trainer utility was based on the statistic alanalysis of pre- and posttreatment data. Results: Anterior crowding was detected in the mandibular arch in 33.8% of children. At the end of the study period, significant changes were observed in the group examined. The prevalence and intensity of crowding diminished statistically (II means from 7.67 to 3.19). Conclusions: The present study demonstrates that the Pre-Orthodontic trainer application gives good outcomes in the early treatment of mandibular primary crowding. (Clin. Exp. Med.
