Background:    Full correction of tetralogy of Fallot (TOF) is a routine procedure that gives excellent long-term results.
    Case Report:    We present a case of a late outcome after complete correction of tetralogy of Fallot. The ongoing heart failure was the result of the pulmonary stenosis, and symptoms were aggravated by obesity and venous insufficiency. Indications for redo-surgery with right ventricular tract reconstruction were established, but the patient did not accept this option.
    Conclusions:    There is a need for follow-up of congenital heart disease in adult life, with increased vigilance for timing of redo-surgery and implementation of new methods of treatment.
