Recurrence of hepatocellular carcinoma in diaphragm and major omentum nine years after initial left hemihepatectomy

Background:    Hepatocellular carcinoma (HCC) is an aggressive malignancy with poor prognosis, mainly determined by recurrence rate.
    Case Report:    Herein, we report on a patient who presented with late recurrences of HCC in diaphragm and major omentum 9 years after initial left hemihepatectomy for liver tumor, previously misdiagnosed as focal nodular hyperplasia (FNH).
    Conclusions:    Our case raises questions about differential diagnosis between FNH and HCC with central scar and negative alpha fetoprotein, and demonstrates the necessity of considering late extrahepatic metastasis of HCC in patients with abdominal mass and history of surgical treatment of HCC or large FNH.
