Kinetic differences of creatinkinase-iso-enzyme (CK-MB) and troponin-T in patients with myocardial damage

Background. Cardiac enzymes are important in detection of myocardial damage. More sensitive and specific parameters than CK-MB might enable for an earlier diagnosis and better therapeutic decision. Troponin-T is offered as a new parameter. The aim of this study was to compare the diagnostic value of CK-MB and troponin-T for my ocardial damage in patients undergoing heart operation. Materials and methods. Normal range was determined in 40 healthy volunteers. The influence of physical stress was monitored in 42 sportsmen before and after exercisetest. Results. No pathologic increase of troponin- T has been observed in healthy volunteers or sportsmen when normal range is set up to 0,1 ng/ml. In 86% of patients after heart surgery the course of troponin-T and CK-MB concentration curves were parallel. Only in 14% as light difference have been seen as far as the signal increase, begin of increase or decrease are concerned. Conclusions: We conclude that there is no pathological increase of CK-MB and troponin-T concentration afterphysicalexerciseinhealthypersons and sportsmen. There is no significant difference in the course of CK-MB and troponin-T concentration after heart surgery, so no diagnostic advantage is assumed by measuring troponin-T concentration compared with CK-MB.
