The influence of malabsorptive bariatric procedure on selected serum atherosclerosis risk factors in morbidly obese patients

Intorduction: Obesity is a major public health problem. Obese persons are at increased risk of morbidity from dyslipidemia, hypertension and cardiovascular diseases, which are closely related to atherosclerosis. The aim of this study was to evaluate the lipid and lipoprotein concentrations in morbidly obese patients before Roux en Y gastric bypass (RYGB) and six months after the surgical treatment. Material and methods: Fifty-one obese patients (28 women and 23 men) prepared to gastric bypass were examined. Twenty-two (15 women and 7 men ) were studied for 6 months after surgical treatment. Forty-eight healthy non-obese persons were evaluated as controls (25 women and 23men). Serum levels of total cholesterol, triglycerides and phospholipids, HDL-cholesterol, lipoprotein (a) were evaluated. LDL-cholesterol was calculated by the Friedewald formula. Results: Obesity is usually connected to the metabolic risk markers of atherosclerosis. The results of our study do not support this hypothesis. Dyslipoproteinemia, especially regarding lipid composition in LDL and HDL molecules, was present in all sera before and after RYGB, when patients’ BMI was significantly reduced. Conclusions: This study confirms the existence of complex at herosclerosis risk factors in morbidly obese patients. To assess this risk properly, lipid composition of lipoproteins should be evaluated on the top of the standard serum lipids and lipoproteins profile. (Clin. Exp. Med. Lett. 2006; 47(3):183-186)
