Operative Treatment Of Grow-up Congenital Heart Disease –
The Own Experience

Introduction: About 30% of all congenital defects in neonates concern the heart and great vessels. In Europe, a rate of 8 to 10 cases of congenital heart disease (CHD) per 1000 alive born children is assumed. In Poland, about 3.000 to 3.800 children with CHD are born each year. In a dozen or so years some of these children will achieve adolescence and will be hospitalized in grow-up centers. Nowadays, the majority of adults with CHD referred to cardiac surgery centers undergoes the defect’s correction for the first time. Material And Methods: All patients with CHD aged 18 or older, operated in the Department Of Cardiac Surgery, Medical University Of Łódź between 1997 and 2002, were analyzed. The above mentioned group comprised 115 patients at the mean age of 37 ± 12 years (from 18 to 66 years). In this group were 74 women (64%) at the mean age of 38 ± 13 years and 41 men (36%) at the mean age of 34 ± 10 years. The biggest subset (n=104; 90,4%) consisted of patients with various types of septal defects, predominantly with atrial septal defect (ASD) of ostium secundum type. Results: All surgical procedures performed in our center in the analyzed period were primary corrections of CHD. During this period we did not re-operate any patient who had previously undergone correction in childhood. Most of the operations (94,8%) required use of cardiopulmonary bypass (CPB) and only 6 patients (5,2%) were operated upon without its application. In 115 adult patients operated upon in our center in the analyzed period there was no case of early postoperative death. Conclusions. In the analyzed period (1997-2002) in our center, only the first defect’s corrections in patients with CHD were performed. The corrections of simple grown-up congenital heart diseases are at the low risk load (no early mortality in our material). The most frequently operated CHD in adults was ASD – 75%. In the near future one should expect that a new subset of adult CHD patients will appear, those who previously underwent the defect’s correction in their childhood. (Clin. Exp. Med.
